Registration/Terms and Conditions

To register, please call the organization's representative number. After confirming your name and address, we will send you a form and platform by mail.

Please take a look at the form and platform that will be sent with your registration form.

Please be sure to use a ballpoint pen or other indelible ink pen to fill out the form.

Once all forms are completed, please return them in the enclosed pre-paid envelope. Please note that the organization will not be responsible for any lost or self-paid envelopes that are returned.

ご登録はの際は、等団体の代表番号に一度お電話ください。ご住所氏名を確認後、郵送にて記入用紙と記入用プラットフォームをお送りいたします。

ご郵送された記入用紙と共に入っている記入用プラットフォームをご覧いただいた上でご記入ください。

必ず、記入用紙はボールペン等の消えにくいものをご使用ください。

全ての用紙に記入が完了したら再度、同封されている納付済み封筒に入れ返送をお願いします。返送する際の、納付済み封筒の紛失、自腹での郵送に関しましては、等団体は一切責任を負いませんのでご了承ください。
